PAAโs Veteran Transportation Program has a mission to ensure area veterans can access their non-emergency medical care at the Hampton VA Medical Center. With over 90,000 veterans living in the Hampton Roads area, access to medical care is critical.
As a Veteran, this self-directed program provides you with the opportunity to access essential services that enable you to stay in the comfort of your home and community. Tailored to meet the needs of Veterans, this program is specifically designed to benefit Veterans of any age who are facing nursing home placement or whose caregivers are overwhelmed. Enrollment in this program is through the VA Medical Center(s).
This program offers Veterans and their caregivers greater access, choice and control over the long-term services and supports (LTSS) that help Veterans live at home and remain a part of their community. Veterans enrolled in VDC decide for themselves what mix of goods and services best meet their needs. This includes the ability to hire, schedule and supervise their workers.
All Veterans, regardless of age, who are enrolled in the VA health care system and meet the clinical need for the service, are eligible to participate
