As someone seeking support, our Care Coordination services offer you, older adults, persons with disabilities, and family caregivers, the chance to explore diverse community options for aging in place. Our dedicated Care Coordinators work closely with older adults to understand their strengths, needs, and preferences. They provide assistance with referrals and follow-up as necessary to ensure a tailored and effective care plan.

To empowering older adults in income-based housing to maintain independence, PAA’s on-site Outreach Workers connects you with essential community resources. The Outreach Worker organizes educational programs, conducts resident assessments, facilitates referrals, and ensures thorough follow-up support.

Occasionally, a crisis can emerge, thrusting you into the role of caregiving without a pre-established plan and understanding the requirements of your loved one can feel overwhelming. PAA offers a wealth of information and resources to aid you in this capacity. As a caregiver, you’ll benefit from an array of programs, such as Family Caregiver Lunch & Learns, Memory Café, support groups, and educational training sessions.

In addition, respite services may be accessible to qualified individuals aged 60 and above through the Adult Day Care and Personal Care programs.

Free Wills Program
As a participant in the Free Wills program, you’ll have access to legal estate planning services tailored just for you. Dedicated volunteer attorneys who will draft a basic will, medical directive and Power of Attorney during your visit. The document is executed the same day, ensuring it’s finalized and effective immediately for your peace of mind

Legal Aid Society of Southeastern Virginia
PAA refers clients to Legal Aid for most civil cases as long as you’re at least 60 years old. Some of the most popular services provided by Legal Aid are Medical Directives, uncontested Divorce and Power of Attorney.
